The machine was stopped and a sprag put up to this prop from the foot of another one, but when the machine, was re-started the prop again moved. The machine, was again stopped and then the roof coal fell and killed him.

Deceased had left three wagons on one line so near the points that they fouled the other line. He must have forgotten them, for he afterwards lowered six wagons on the other line controlling them by the front brake. His back was towards the set of three and he was caught between the sides of the two sets as they grazed each other.

STREET Joseph 31 18/12/1913 Shot firer Clay Cross He was examining the connections between shot-firing cables and the detonator leads before firing a shot, when 3 tons of coal fell away from the face and killed him. The sprags had been taken out 3 hours before, but no attempt was made to get the coal down, and it did not fall, the sprags were replaced. A careful examination might to have shown that blasting was not necessary.
